NLE Passers July 2011 Registration Schedules

The PRC Board of Nursing (BoN) via Honorable Marco Antonio Sto. Tomas on Facebook has released the Registration Schedule for Lucena City - NLE licensure passers last July 2011.

The schedule currently available is for Lucena City only. The NLE July 2011, registration starts September 1 to October 17, 2011, weekends not included.

Requirements

1. Duly accomplished Oath Form or Panunumpa ng Propesyonal. Download the official / updated form here.
2. Current Community Tax Certificate (cedula).
3. Two (2) pieces passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) piece 1”x1” picture in plain background with complete name tag (for Registry of Professionals).
5. Two sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with name and profession.

We however apologoize for the low resolution screenshot, we will be updating this post for a high resolution form as soon as we get our hands on it.
